We’re Hiring: Clinical Care Coordinator

Location: Mt Laurel Welsh Campus - Lindens

Employment Type: Non-exempt Full-Time, In-Person

Department:Lindens

Reports to: Clinical Director

Hourly Rate: $23.65-$26.92

 

The Clinical Care Coordinator provides clinical coordination and support across interdisciplinary teams and external team members to ensure effective service delivery. Maintains schedules, documentation, and communication to facilitate effective treatment.

Responsibilities

  1. Coordinates between team members and disciplines to allow development and implementation of interdisciplinary treatment for individuals
        • Reviews, monitors, and maintains any necessary clinical schedule and collaborates with clinical team members to balance the needs of the program with those of the clinical team.
        • Maintains internal documentation to help team members understand their roles and responsibilities, caseload breakdowns, and other internal resources.
        • Actively maintains therapeutic treatment schedule and ensures collaboration among team members. Coordinates with key team members to ensure effective treatment services.
  1. In support of treatment plans, identifies training needs of Program and staff members. Coordinates and conducts training sessions to ensure the ongoing knowledge and ability of staff members to effectively implement behavior plans.
  2. Oversees the observation of persons served and completion of treatment integrity checks to track and document staff behavior and provides feedback as necessary to the appropriate team members.
  3. Ensures and facilities requirements for high restraint users, including but not limited to, identifying high restraint users and documenting ongoing conversations about treatment.
  4. Communicates with staff on an ongoing basis to promote a constant flow of information, open discussions, knowledge sharing, and to keep staff well-informed. Attends various internal management meetings, etc., to both obtain and share information.

Qualifications

Bachelors Degree Required in related field
