Overview

The Funding Coordinator will work collaboratively with Bancroft Leadership, Clinical staff, Finance Staff, and external case manager/funders to ensure the individual served accepted to admission meet and maintain funding eligibility, maintain the authorizations, and obtain approvals for needed services.

Responsibilities

Essential Responsibilities:

 

  1. Manage aspects related to assigned caseload that pertain to access to Bancroft services, funding and ensure cases are approached in a thoughtful, holistic manner that reflects Bancroft’s core values. 
  2. Track and maintain current authorizations for individuals.
  3. Maintain effective communication with the treatment team,external funder, to justify any changes in services that are requested. 
  4. Ensure that all documentation for the annual plan is promptly prepared and provided to the appropriate parties. 
  5. Utilize the change form process to communicate with billing and finances any changes in services and insurances.
  6. Utilize the re-authorization process for securing authorization for MLTSS.
  7. As needed facilitates eligibility and benefit checks and oversees the process of redetermining and/or restoring eligibility for benefits and entitlements. 
  8. Assist with securing funding authorizations for durable medical equipment and adaptive devices. 
  9. Assist the individuals with obtaining disability benefits when needed
  10. Ensure that all pertinent contact information for each individual and his/her care team and relationships is accurately maintained in the electronic database (Salesforce). Enters discharges and transfers into Salesforce.
  11. Oversee the electronic files for assigned caseload.
  12. Assist families, individuals served and residential care teams with resolving medical insurance issues.
  13. For new admissions, attend the pre-admission and admissions meetings to meet the external care team members and to learn about the service needs of the individual being admitted.
  14. Manage service utilization to ensure that individuals services are being delivered
  15. Maintain a clear understanding of the funding regulations relevant to assigned caseload.
  16. Maintain a working knowledge of all Bancroft Policies and Procedures relevant to the Case Coordinator position.
  17. Provide training to managers and others as needed.
  18. Serve as a resource for program managers and directors to resolve random, unassigned issues that impede the individual’s access to service, health and overall progression.
  19. Serves as a role model by projecting a positive self-image of professionalism, appearance, confidentiality, courtesy, conduct, honesty, fairness, personal integrity, and a respect for the fundamental rights, dignity, and privacy of others.
  20. Abides by the Bancroft Code of Ethics, Mission Statement, and Vision Statement in promoting ethical behavior, establishing relationships and providing guidance in decision-making situations
  21. Remains current with required training certifications, meets state-regulated licensing and regulatory accreditation, and adheres to mandatory requirements.
  22. Maintains effective and collaborative verbal and written communications with colleagues, those served, external case managers and family members and/or guardians.
  23. Willingly and effectively cooperates with Bancroft, The Department of Human Services, The Division of Developmental Disabilities (DDD), and other licensing or state agencies or local municipalities in any inspections, audits and investigations, upon request.
  24. Maintains a safe and respectful environment, free of abuse, neglect, or exploitation; does not allow weapons, threats, bullying or intimidation.

Qualifications

Education & Experience:  Bachelor’s degree required.  Minimum of two (2) years’ experience of related experience in case management and family advocacy required. 

 

Special Skills:

Proven ability to effectively communicate both verbally and in written form required.  Effective and proven ability to prioritize, multi-task; management skills required, as well as flexibility and adaptability. Ability to assess situations, problem solve and delegate tasks. Effective interpersonal and organizational skills. Basic math skills required. Operative knowledge of MS Office Word and Excel spreadsheets preferred.  Valid driver’s license required in incumbent’s legal name and current address with no provisional restrictions. Minimum 18 years of age required.
