Responsible for medication administration, attending medical appointments as needed, and serves as a medical  liaison with healthcare providers, program management, etc., on behalf of assigned person(s) served.

• Administers medication(s) as per regulations of the Department of Human Services: Division of  Developmental Disabilities (DDD), and following all applicable protocols. 
• Reviews MAR’s for each person served at least once weekly of the Medication Administration Record,  prescriptions, and medication(s) along with nursing and submits review to the supervisor. 
• Reports variations from the norm to the on-site Licensed Practical Nurse or Registered Nurse (as  applicable) and program manager if appropriate.
• Conducts supplies inventory as directed by supervisor for health team and residential first aid kits and OTC  boxes.
• Prepares Medication Administration Records ( MAR’s) for all persons served in the Lakeside program every  month. 
• Monitors inventory of all medication for all person served ensuring the medication supply not run out: a. Coordinates and checks in bi-weekly pharmacy delivery.
• Is on-site to coordinate the distribution of new medications to the group home.
• Responsible for disposing medications as directed by supervisor and following Bancroft Policy and  Procedure. 
• Ensures Medication storage areas are well maintained.
• Monitors documentation of medication/medical related items in the critical log and missing entries are reported to the manager and followed upon.
• Oversees that the MAR binder are maintained and only current scripts are inserted and that the OTC sheet  is current.
• Prepares assigned person(s) served for all medical appointments with physicians, dentists, and healthcare  specialists; e.g., washing, bathing, grooming, appropriate attire, etc.
• Ensures the accuracy, completeness, and currency of all medical information maintained at the program  site for assigned person(s) served in their caseload as directed by the Health Care Coordinator;
• Prepares for individual medical appointments for assigned person(s) served; e.g., confirms insurance and  funding information prior to visit, ensures that all necessary pre-testing has been performed and results  received; e.g., laboratory tests, x-rays, special studies, etc.
• Personally accompanies assigned person(s) served to medical visits or makes arrangements for a proxy;  communicates with healthcare providers on behalf of person(s) served, providing medical history, explaining  current health status, behaviors and reason for visit, providing details regarding medications and reactions,  etc.
• Performs necessary follow-up post-medical visits; alerts program site staff of any medication changes;  observes and monitors person(s) served post-administration of newly-prescribed drugs and promptly  notifies healthcare providers and program management in the event of an adverse reaction; appropriately  updates and files medical information for assigned person(s) served.
• Provides supervision and direct care to assigned person(s) served in the event of medical or dental  procedures, etc., which may vary from short duration to extended periods of time or until relieved; 
• Serves as liaison with healthcare providers, insurance providers, equipment suppliers, etc., in arranging for  and acquiring various medical equipment and supplies for assigned person(s) served; 
• Serves as an advocate on behalf of assigned person(s) served in a variety of situations. 19. May perform other duties as requested by Supervisor/Program Leadership. 
• Maintains effective communications with assigned person(s) served, parents, guardians and other family  members, healthcare and insurance providers, own manager and coworkers across programs and  functions, Nursing staff, etc. Operates in accordance with the Confirmation of Understanding for assigned  person(s) served regarding programs and treatment, healthcare, ethnic and cultural support, physical  fitness and healthy living, family visits and involvement, and disagreements and grievances.
• Manages medication list via online and other means as applicable, including, but not limited to: Copy  medication list to provide for IHP's, IDT's, parents, auditors, surveyors, etc.; Confirm that the MAR is correct;  Confirm that pharmacy has received scanned copies of orders if there is a discrepancy during an MAR  audit. 
• Audits menus in assigned residence(s) to ensure compliance with regulatory guidelines and special dietary  orders for any/all residents in the home(s).
• Projects a positive self-image of professionalism, appearance, confidentiality, courtesy, conduct, honesty, fairness, personal integrity, and a respect for the fundamental rights, dignity, and privacy of others.
• Performs other related duties as necessary and dependent upon Program supported; e.g., may participate in special projects, etc. Remains flexible and adaptable in work schedules and work assignments as defined by Program needs.
• Abides by the Bancroft Code of Ethics, Mission Statement, and Vision Statement in promoting ethical  behavior, establishing relationships and providing guidance in decision-making situations. Follows Bancroft Policies and Procedures. 
• Remains current with required training certifications, meets state regulated licensing and regulatory accreditations, and adheres to mandatory requirements.
• Maintains effective verbal and written communications with colleagues, those served, and their family members and/or guardians when applicable.
• Willingly and effectively cooperates with Bancroft, The Department of Human Services, The Division of Developmental Disabilities (DDD), Division of Children and Families (DCF) and other licensing or state agency or local municipalities in any inspections and investigations, upon request.
• Maintains a safe and respectful environment, free of abuse, neglect, or exploitation; does not allow weapons, threats, bullying or intimidation. 
• Reports any violations to the appropriate individual as soon as the incident occurs.
• Maintains levels of support as defined in behavior plans; e.g., Individual Program Plan (ISP), Individual Rehabilitation Plan (IRP), Individual Education Plan (IEP), Individual Service Plan (ISP), etc., when applicable.

Education & Experience: 

High School Diploma or equivalent with a minimum of one year experience working with individuals with  developmental disabilities required.  

Special Skills: 

Demonstrated ability to effectively communicate both verbally and in written form required. Basic math skills  required. Effective interpersonal skills, a strong desire to work with neurologically-challenged individuals required,  and the ability to implement crisis intervention techniques required. Operative knowledge of MS Office Word and  Excel spreadsheets preferred. Attainment of CPR, First Aid, Crisis Prevention and Intervention, and Medication  certification within first 90 days of employment required. Valid driver’s license required in incumbent’s legal name  and current address with no provisional restrictions. As well as one year minimum of driving experience with the  required license. Minimum 18 years of age required.
