We provide a full continuum of highly effective services — for people of all ages and every level of abilities — based on best practices and scientifically proven techniques. Our services include special education, vocational training, supported employment, structured day programs, group home and apartment programs both on-campus and in the community, short-term behavioral stabilization services for children, and in-home and outpatient rehabilitation services. We are currently looking for a Social Worker for our Children's Program in Mt. Laurel, NJ.

Responsibilities

Essential Responsibilities:

 

Operating in accordance with regulations defined by the N.J. Board of Education, Division of Developmental Disabilities (DDD), Commission on Accreditation of Rehabilitation Facilities (CARF)and Health Insurance Portability & Accountability Act (HIPAA), Office of Children’s Advocacy standards, established Bancroft policies and procedures,applicable state/local/federal regulations, each student’s Individual Program Plan (IPP) and specifically the student’s Individual Education Plan (IEP), serves as primary liaison between the School, social services, agencies, family members, and residential sites to provide social work services, case management, and support services for students and their family members.

 

Interacts with Education staff, residential program management, family members, etc., to ensure all IPP/IEP materials are received on a timely basis from appropriate parties and are available to implement and monitor the plans for each individual student;On an ongoing basis and in conjunction with Education management, education faculty, clinical-specific staff, and family members, monitors existing plans to ensure that stated educational goals and objectives are appropriate, achievable, and continue to meet the needs of individual students;Actively participates in and recommends appropriate modifications to the existing education plans to better address and document the educational and (if applicable) residential goals and objectives for each student;

 

Acts as liaison between the Education program and students, family members, fund sources, Admissions Department (if applicable), and future discharge sites to ensure an efficient admission and discharge process.  Interacts on a regular basis with school districts, N.J. Division of Child Protection and Permanency (DCP&P), other state agencies, etc., regarding various aspects of program participation; e.g., prepares correspondence, completes required forms, etc., performs necessary follow-up activities, etc.; 

 

Convenes, coordinates, and participates in the Interdisciplinary Team (IDT) process in conjunction with the IPP.  Attends Interdisciplinary Team (IDT) meetings to both provide and obtain information regarding students, review baseline goals, identify individual strengths and weaknesses, assess student progress regarding attainment of goals, determine the need for modifications to existing goals, combine goal teaching strategies for best possible outcome, and discuss concerns regarding specific students to ensure overall IPP effectiveness within the educational environment. 

Attends various student review meetings, team/departmental meetings, etc., as needed, to review, discuss, and serve as student advocate at major decision points throughout students’ participation in the Bancroft education system, pending transition into other Bancroft programs, etc.

Qualifications

  • A minimum of one-year experience working wih youth and I/DD 
  • Master's Degree in counseling, social work, psychology or a related field required
  • A license to practice independently in NJ inlcuding LCSW, LMFT, LPC or Masters Level therapist licensed to practice in NJ including LSW and LAC who will achieve full independent licensure in the timeframe required by the approriate licensing board and is practicing under the supervision of NJ independently licensed therapist. Supervision must be in accordance with corresponding licensing board regulations
